A little introduction as to what Caswell has on offer for patients. There's an opportunity for patients to participate in their care and write their own CTP report, and to say what's important to them as service users, where they see themselves in the future, in relation:

•  FRIENDS
•  FAMILY
•  COMMUNITY

Also which goals they set themselves and how Caswell can help you achieve these goals. There will be workshops running across the hospital to help give patients the skills and opportunities to do this
